Your permission I have read the privacy notice. I agree to my photograph being processed to create this cosmetic skin reading, including by the analysis provider described in the privacy notice. I agree to DR SKNN matching my details to an existing clinic record in Pabau, or registering me if I’m new.

Contact about your reading · required

We need to be able to reach you about your reading, including if a clinician identifies something that needs follow-up.

I confirm these are my contact details and understand that DR SKNN may phone, text or email me about this reading, any concern needing follow-up, or a consultation I request. This does not sign me up for marketing.

A skin reading is not an emergency or diagnostic service. Contact the clinic or your usual healthcare service if you have a concern; do not wait for us to call.

WhatsApp for your reading · optionalYou may also contact me on WhatsApp about my reading or consultation.

Phone, SMS and email follow-up remain available if you leave this unticked.
